About
Kind Heart Personal Care Home provides protective care and watchful oversight for seniors who desire to maintain an
active lifestyle but need some degree of supervision and assistance with daily personal needs. Adults must not have
ongoing medical needs, like IV fluids, that typically require a higher level of care such as found in Skilled Nursing
Facilities. By not being an “institution” but a residential home in the community, we offer “Alternative Living Services” in
Columbus, Muscogee, GA and other surrounding counties in the River Valley Region of GA. We are licensed and
regulated by Georgia Department of Community Health, our staff complete background check before they are selected
to become a care giver in our home.
Our Services
- Bedroom
- Meals: Three per day plus snacks
- 24-Hour Supervision and Watchful Oversight
- Medication Management: Includes supervision of self-administered medication
- Companionship
- Personal Care Services: Assistance with dressing, eating, bathing, grooming, and toileting
- Activities and Recreation
- Housekeeping and Laundry Service
- Provide or Arrange Transportation: And other services in the types and amounts needed by the resident
- Assistance with Referral to Appropriate State Agencies and Programs
- Services Supervised by a Registered Nurse
Please note that Kind Heart Personal Care Home administrator will speak with the client family, physician, and/or case manager and conduct a pre-admission assessment. Kind Heart Personal Care Home does not have the capability to serve persons with:
- Complex medical and behavioral health needs
- Residents who require bed to wheelchair transfers and vice-versa using a Manual Hoyer-Lift or two (2) aids/assists on transferring or ambulation.
Quality Assurance
Survey or inspection reports are one measure to determine the ability of a nursing facility to deliver quality services. Although surveys and inspections are tools to use when comparing nursing facilities, the best measure of quality is your own opinion. Visit our facility at any time. How Individuals Can Pay for and Get Government Assisted Care At KHPCH
KHPCH cost is very reasonable and competitive; this facility is excellent choice for seniors seeking quality living at a reasonable cost.
KHPCH accepts both
Self-pay / Privately funded clients and those
Medicaid Payments funded by Elderly Disabled Waiver Program (CCSP and SOURCE)
Both Medicaid Waiver Programs – Community Care Services Program (CCSP) and Service Options Using Resources in Community Environments (SOURCE) are case management programs under the Elderly and Disabled Waiver Program (EDWP) administered by the Department of Community Health.
Medicaid Waiver CCSP / SOURCE Eligibility:
In order to qualify for assistance through CCSP and SOURCE, the individual applies through his / her local Area Agency on Aging (AAA) – An Area Agency on Aging (AAA) is a public or private non-profit agency, designated by the state to address the needs and concerns of all older persons at the regional and local levels. The individual must meet the following:
Deemed appropriate for nursing home level of care by a physician and have a need for the services
Medicaid eligible or potentially eligible based on a financial screening
Able to safely live in the community with supporting services
The Contribution of Individuals to Government Assisted Care at KHPCH
Contributions to CCSP / SOURCE – For individuals receiving Supplemental Security Income (SSI) benefits, there is no fee for services. For the other participants who qualify for Medical Assistance Only Medicaid, there may be a monthly fee for certain services.
